Job Overview

We are offering an exciting opportunity to join our Older Persons and Physical Disability Teams as a Level 2 Registered Social Worker based in the Staffordshire Moorlands.

The service supports people of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

We are looking for a highly skilled professional with excellent communication and organisational skills and working knowledge of the Care Act 2014 and Mental Capacity Act 2005.

The role emphasizes professional development and career progression within an integrated organisation offering learning opportunities such as Practice Educator, Consultant Social Worker and AMHP.

Responsibilities
  • Play a key role in supporting the delivery of the Personalisation and Inclusion Pathways for Adults and Carers, ensuring legal literacy, strength and asset‑based practice, and social inclusion is embedded and applied effectively.
  • Carry out a range of assessments, including holistic needs‑led assessments, complex care and support planning and reviews, and, where appropriate, initiate a multidisciplinary assessment.
  • Have comprehensive knowledge of the services available to support and promote independence in the home environment.
  • Provide strength‑based practice, forging links within local communities to deliver an outcome‑focused, person‑centred service as the system moves toward a meaningful integrated care system.
