A regional healthcare trust in the United Kingdom is seeking a dedicated professional to assess patients in their homes, addressing acute illnesses and facilitating their recovery. This unique role requires relevant experience in a direct care setting and a full UK driving license. Team members will work collaboratively in a nurse-led environment and must be dynamic and flexible. This role involves assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short‑term support to remain at home.
The service is fast paced and you will encounter a wide range of patients with diverse conditions. It is a unique and rewarding role that enables you to develop your clinical skills while working directly with patients in their usual place of residence.
The Recovery at Home team is nurse‑led and works with a multidisciplinary team that includes Nurse Practitioners, Nurses, Occupational Therapists, Physiotherapists, Health Care Assistants, Therapy Assistants/Exercise Practitioners and Administrative staff. It also collaborates closely with partner agencies in the local voluntary sector.
