Overview

This role involves assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short‑term support to remain at home.

The service is fast paced and you will encounter a wide range of patients with diverse conditions. It is a unique and rewarding role that enables you to develop your clinical skills while working directly with patients in their usual place of residence.

Responsibilities
  • Conduct rapid assessment and treatment of patients in the home with acute illness or injury.
  • Provide short‑term physical and functional support to allow patients to remain safely at home.
  • Work 24/7 to prevent admission or readmission to hospital.
  • Use pool cars for completion of patient visits.
  • Support learning and professional development in a collaborative environment.
Qualifications and Requirements
  • Dynamic, motivated, enthusiastic and flexible in approach.
  • Relevant experience in an acute care or community environment.
  • Full UK driving licence.
  • Proactive learner who meets identified time‑scales.

Team and Support

The Recovery at Home team is nurse‑led and works with a multidisciplinary team that includes Nurse Practitioners, Nurses, Occupational Therapists, Physiotherapists, Health Care Assistants, Therapy Assistants/Exercise Practitioners and Administrative staff. It also collaborates closely with partner agencies in the local voluntary sector.
