Job description
Job Overview

Lead on the day to day operational management and delivery of a high quality Hospice at Home service for patients and families with complex palliative care needs who wish to be cared for at home. Act as a point of reference within specialist palliative care providing support and advice to Health and Social care professionals within Primary and Secondary care to ensure that patients/carers access the most appropriate service. Take responsibility for reviewing and prioritising referrals to Hospice at Home and allocate for home visit/risk assessment/care shift as required, or signpost to more appropriate service depending on needs. Responsible, alongside job share colleagues, for the line management of the Hospice at Home Health Care Assistants, including carrying out supervision, appraisal and Personal Development Reviews with these staff. Rotational on call cover 9am - 5pm at weekends/Bank Holidays is a requirement of this post.

Main duties of the job
  • Manage and maintain day to day operations of Woodlands Hospice at Home service in line with Commissioners specifications.
  • Lead and manage the day to day organisation and co-ordination of Woodlands Hospice at Home.
  • Demonstrate understanding and competence in clinical skills appropriate to role, including holistic assessment of patient's nursing needs. Plan and implement effective service delivery according to changing health needs and varying levels of complexity for patients requiring Hospice at Home service.
  • Work collaboratively with other professionals and agencies to ensure an effective service and that patient needs are being met. Organise service making best use of resources in response to referrals received.
  • Liaise with patients and families using the service and where relevant undertake domiciliary visits to assess the situation, and involve patients and carers in the planning and delivery of care.
  • Establish and maintain effective communication with patients and carers including sharing sensitive news.
  • Collect, collate, evaluate and report information, maintaining accurate patient records.
  • Participate and actively contribute in multi-professional meetings and ensure agreed actions are taken.
  • Provide and receive appropriate information concerning patient and family to relevant health professionals involved in their care.
  • When required for holiday/sickness or other absence cover, assume the role of the ASPCT Triage Coordinator, coordinating patient crisis prevention/intervention needs in response to contacts from patients/carers and social or healthcare professionals.
  • Recognise and respond appropriately to urgent and emergency situations.

Qualifications

  • Registered Nurse / Midwife
  • A recordable qualification in Palliative Care or equivalent
  • Evidence of continued professional development

Experience

  • Demonstrable post registration experience within a palliative care clinical setting in either the community, hospice or acute hospital.
  • Experience of managing and organising a clinical area
  • Experience of staff development and line management

Knowledge

  • Knowledge of symptom control, end of life care and palliative care emergencies.
