A community health organization in Shrewsbury is looking for dedicated healthcare professionals to join their urgent community response service. This role involves delivering quality healthcare interventions to patients in their own homes, promoting independence and wellbeing. Suitable candidates will possess experience in healthcare delivery and a valid driving license. The position requires building strong relationships with healthcare teams to ensure high-quality patient care, and it operates 7 days a week.
Please note the Trust will not routinely provide sponsorship to work for this role. To meet the Home Office salary threshold you must have the significant relevant experience for this role to be offered the top of the pay band. An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards Service. The post will form part of a seven day service based. Vacancies are available in the following locality teams: Shrewsbury, Telford, Bridgnorth, Church Stretton, Whitchurch, Oswestry.
The service provides patients who are experiencing an acute change in their health, a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.
The service offers an appropriate alternative to admission pathway, which aims to deliver a recommended and agreed medical, nursing, and therapeutic treatment plan. The focus is supporting the safe management and provision of care for people in their own home. The role and service offer integration and partnership working across the system to ensure a seamless service.
Please note this position is community based, requiring the successful applicant to see patients in their own homes. Therefore, a valid driving licence with access to a vehicle for work purposes is essential.
You will be responsible to support the delivery of quality healthcare interventions to patients living in their own environment in the community, in order to meet their health and wellbeing needs, and to facilitate re-ablement support in order to maximise a patients functional independence at point of referral.
You will support individuals and clinical teams to deliver high quality, clinically effective patient care through assessment, planning, implementation and evaluation. Along the way, you'll build strong relationships with a wide range of healthcare professionals and patients.
Operating hours of the service will be 08.00-00.00, 7 days a week.
