Job summary

Qualifications

We are looking for a HealthcareAssistant to join our Hospice @ Home Team to assist with the delivery ofholistic care to patients, family/carers with life limiting illness, withintheir own living environment.

Thesuccessful candidate will be subject to a DBS Check at the level appropriate tothe role, the cost of which is covered by the hospice. Any post holder withinthe organisation will be expected to undertake safeguarding trainingappropriate to their role and adhere to safeguarding policies and procedures.All staff must work in accordance with their statutory roles andresponsibilities in relation to safeguarding in accordance with the WorkingTogether to Safeguarding Children 2023, The Care Act 2014, and Prevent Duty2015.

Main duties of the job
  • Playa key role in the provision of specialist palliative and supportive care.
  • Involvepatients and their families first and always go the extra mile to provide firstclass care.
  • Playan integral part of the multidisciplinary team and have strong workingrelationships with other health care providers.

Whilstyou will have a specific base, there may be occasions where you are required toprovide cross-site/service cover.

Job responsibilities

You will work aspartof a compassionate, multidisciplinary team, working in partnership withthe patient, family & carers.

You will:

  • Assess,plan and deliver holistic, person-centred care.
  • Supportpatients and carers physically, emotionally and spiritually.
  • Keepaccurate records and communicate clearly with the wider care team.
  • Use your knowledgeof local services to ensure the best support for every family.
  • Occasionallyprovide cross-site cover when needed.
Person Specification
Qualifications
  • Have NVQ level 2 in Care or equivalent in Care
  • Or working towards NVQ level 2 in Care or equivalent
  • Or willingness to undertake NVQ level 2 in Care or equivalent
  • Be willing to undertake Compassion in care course
  • Hold a full driving license and have access to a vehicle with car insured for business use
  • Have the right to work in the UK. We are not able to offer sponsorship at this time
Experience
  • Experience working in palliative care
  • Previous hospital experience or experience of working in a similar care setting.
  • Demonstrate an interest in specialist Palliative care
