Job description

We are looking for forward thinking, motivated Health Check Assistants who have a passion for new ways of working whilst providing safepatient and effective care.

Job Type: Bank Staff hours flexible (annualleave/sickness cover 2 shifts per month minimum)

Operates: The service operates 7 days a week, working hours vary day to day

Pay: £12.45 perhour with enhancements for weekend and bank holidays

Main duties of the job

The successful candidate will work providing health checks andoffering lifestyle advise to patients across Lancashire. Observations includeSP02 levels, heights, weights pulse rates, BP checks, point of care testing forcholesterol and pre diabetic screening where appropriate. This information willbe inputted in real time to the EMIS IT system including calculation of theQRisk3 score.

Full training for point of care testing will be provided and othersystems where required.

Job responsibilities

Roleof the Service

As part ofour diagnostic business unit, we are working across various locations inLancashire to provide NHS Health Checks to eligible patients aged 40-74. Aspart of this check, patients have observations taken as well as Point of Caretesting for cholesterol and when appropriate HBA1c levels are checked. Forthose not eligible, patients are offered a mini health check for observations.All patients are given lifestyle advice and when appropriate signposted toother services. These health checks can take place in various locationsincluding on our mobile treatment unit, local community venues, shoppingcentres and workplaces.

Dutiesand Responsibilities

  • To undertake NHS health checks in the community face to face with patients.
  • To provide patient education and health promotion.
  • Work to attain the service KPIs and targets.
  • Ensure all patient contact is recorded in a timely manner on the IT systemsto enable tracking of patient outcomes, both in real-time to support ongoingintervention planning and retrospectively to understand the impact of theservice.
  • To ensure mandatory training is kept up to date.
  • Acknowledge and accommodate barriers tocommunication and understanding including speech, hearing, language, andemotion.
  • Flexibility in attending organised educationalupdates and eagerness to expand on current knowledge base and cascading neweducational information to colleagues whenever possible
  • Inform the Clinical Manager of any untowardincidents, work within all relevant policies and procedural guidelines e.g.infection control, safeguarding and risk management.

Educationand In Service Training

  • Be fully conversant in all the clinicalcomputer systems used within FCMS.
  • Attend relevant study/induction days,seminars, courses etc, for individual development and for the benefit of theservice.
  • Wherenecessary relevant training in the operation of new or unfamiliar equipment,software or procedures will be provided or arranged.

On no account must such information bedivulged to anyone who is not authorised to receive it. Confidentiality of information must be alwayspreserved whether at or away from work. FCMShas in place a Whistle blowers Policy for staff wishing to express concerns.
