Overview

As a Health Care Assistant, you will work alongside our diverse team comprising nurses, paramedics, physiotherapists, occupational therapists and technical instructors. We provide evidence-based nursing care to patients in their own homes. Our service operates 8am – 10pm, 365 days a year. We offer flexible working patterns and are happy to consider full or part-time applicants. We support staff through training and development, including time for training, university attendance and placements. Based in Welwyn and Hatfield Integrated Community Team, this role offers opportunities to progress, for example toward Nursing Associate training and Registered Nurse top-up with the support of Hertfordshire Community NHS Trust. A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Responsibilities
  • To work within a team that provides an assessment and care service to patients with nursing/health needs.
  • To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan. This includes aspects of personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists.
  • The aim is to rehabilitate and maximise the patient’s potential and, where appropriate, to support the carer.
  • To support the Hospital at Home team with blood sampling.
  • To support and enable patients in a range of activities to promote independent living and to monitor their well-being.
  • To undertake administration and clerical tasks associated with the care process, including record keeping and use of computer.
