Job summary

A prominent healthcare provider in Worthing is looking for a GP Practice Pharmacist to join their team. In this role, you will improve medicines optimisation and patient outcomes while working collaboratively with healthcare professionals. Qualifications

  • Minimum 2 years experience working in patient-facing pharmacy roles.
  • Good clinical knowledge of medicines used in primary care.
  • Strong communication and interpersonal skills.
  • Ability to work independently while recognising own limitations.
  • Experience working in general practice or primary care networks.
  • Completion of, or working towards, Primary Care Pharmacy Education Pathway (PCPEP).
  • Experience conducting structured medication reviews.

Responsibilities

  • Provide clinical medical reviews for chronic disease management.
  • Conduct medication reviews for patients with long-term conditions.
  • Support the Clinical Pharmacy team in delivering medicines optimisation.
  • Assist with prescription queries from patients and healthcare professionals.
  • Participate in clinical audits and quality improvement projects.
  • Liaise with multidisciplinary team for medicines advice.
