A leading healthcare provider in London seeks a Senior Therapist for a full-time position in the Frailty Virtual Ward. The ideal candidate will manage complex patient cases, work with a multidisciplinary team, and contribute to service development. An exciting full-time 12 month fixed term opportunity has arisen to join a new, innovative and dynamic team within our expanding Frailty Virtual Ward and Rapid Response Team. We are looking for an experienced and motivated senior therapist (Band 7) who is passionate about service development and providing high standard of patient-centered care. You will be working closely within a Multi-Disciplinary Team of staff including Doctors, senior practitioner nurses, Occupational Therapists and Physiotherapists, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. At times this post may require you to work autonomously, when working across the Hackney borough into patients own homes and in an inpatient hospital setting. This post will be offered as a full-time, fixed term (12 month) contract.
