Qualifications

• Minimum of 2 years post qualification experience.
• Experience of working within general practice.
• Independent prescriber or intent of gaining qualification.

Responsibilities

• Undertake structured medication reviews with patients.
• Contribute to multidisciplinary team meetings.
• Provide support with prescription and medication queries.
