Overview

London North West University Healthcare NHS Trust (LNWUH) Diabetes Nursing Service is seeking an experienced and dynamic Diabetes Specialist Nurse to join our team. Based at Northwick Park Hospital, this Band 7 Fixed Term, Maternity Leave Cover requires a highly skilled nurse with extensive knowledge and understanding of diabetes care.

You will work flexibly and proactively, both independently and collaboratively, as part of a small, friendly, and supportive specialist nursing team. Your role will focus on delivering exceptional outpatient and inpatient diabetes care and utilising innovative virtual ward pathways designed to reduce hospital admissions and shorten length of stay.

The post holder will require excellent communication and clinical skills, with previous experience delivering high-quality care for people living with diabetes. A strong commitment to ongoing education is essential to ensure the service remains up-to-date with the latest developments in diabetes management. Main duties of the job

We are seeking an experienced and dedicated Diabetes Specialist to join our multi-disciplinary team providing Tier 4 diabetes services to adults and young people aged 16-19 years. This is an exciting opportunity to play a key role in delivering expert diabetes care, education, and service development within a dynamic acute hospital setting.

Key Responsibilities:

  • Collaborate as part of the diabetes multi-disciplinary team to deliver high-quality Tier 4 diabetes services.
  • Participate in patient education initiatives such as DAFNE to enhance understanding and management of diabetes and its complications.
  • Foster effective partnership working between the acute hospital and the wider multi-disciplinary team to ensure seamless patient care.
  • Provide expert clinical care, particularly in diabetes technologies including insulin pump therapy.
  • Deliver diabetes inpatient services and support remote digital platforms such as Diabetes Virtual Wards.
  • Contribute to the ongoing development of diabetes services in line with NICE guidelines, GIRFT, and Trust initiatives.
  • Work closely with medical and multi-disciplinary colleagues to provide comprehensive, patient-centred care.
  • Participate in teaching programmes for patients, carers, and acute care staff.
  • Maintain and develop clinical expertise through continuous professional development, audit, and education aligned with national diabetes audit requirements.
