Overview
An opportunity has arisen for a part time Community Diabetes Specialist Nurse to join the existing team to provide care to people registered with a GP in Bexley. We are seeking a motivated, enthusiastic individual who is committed to developing and improving diabetes services, ensuring patients receive a high‑quality service in the Bexley locality.
Key Responsibilities
- Support nurse led clinics, multi‑disciplinary meetings and home visits.
- Support patients, relatives and carers via education programmes tailored to individual needs and advice.
- Provide specialist expertise and education in diabetes care to GPs, practice nurses, patients, relatives, carers and other health professionals.
- Prioritise workload and undertake nurse led clinics within the community.
- Work autonomously, frequently travelling off‑site and to patients’ homes.
- Offer timely advice to other health professionals as required.
- Monitor and improve standards of care in accordance with local and national guidance.
- Coordinate staff activity as required and ensure risk management for self, patients, colleagues and the environment.
- Participate in clinical supervision regularly and advise on non‑medical prescribing if undertaken.
- Contribute to development of guidelines, protocols, audit, evaluation and service improvement.
- Use effective communication, teaching and advanced teaching skills to engage patients from diverse backgrounds.
Qualifications & Attributes
- Registered nurse with an active NMC registration.
- First‑level degree or equivalent portfolio of experience.
- Recognised diabetes qualification or keen interest in diabetes management.
- Full UK driving licence and access to a car.
- Excellent communication skills to empower and educate patients, community and practice staff.
- Ability to work independently and as part of a multi‑disciplinary team.
- Non‑medical prescribing desirable but not essential.
- Experience or evidence of complex diabetes caseload management is advantageous.

Professional Responsibilities
- Be professionally and legally responsible.
- Work alone in community settings independently of multidisciplinary support while in direct contact with patients and carers.
- Responsible for assessment, monitoring, management and review of complex diabetes caseload.
- Use effective clinical reasoning and evidence‑based practice to develop individual care plans.
- Maintain up‑to‑date clinical written records and electronic database.
- Coordinate staff activity to meet demands of service.
- Ensure risk is managed for self, patients, colleagues and the work environment.
- Participate in development of the team by working proactively within the MDT.
- Coordinate health‑care needs of people with complex or multiple long‑term conditions.
- Participate in clinical supervision regularly either by self or in a team environment.
- Use clinical skills for assessing and treating patients with diabetes using relevant protocols.
- Identify, monitor and review patients with diabetes in community and nurse‑led clinics for symptom control, prescription and medication titration according to guidelines.
- Work closely with family and GP practice to maximise patients’ health potential.
- Deliver evidence‑based practice in line with clinical governance, trust policies and NMC guidance.
- When available undertake or have undertaken the Non‑Medical Prescribing Course.
- Participate in development of guidelines, protocols, audit and evaluation, and seek views of others.
- Effectively communicate complex and sensitive information to patients and carers, overcoming barriers.
- Use advanced communication and teaching skills for culturally diverse backgrounds, employing interpreters as necessary.
Other Information
Oxleas NHS Foundation Trust provides a wide range of NHS healthcare services in community and secure settings. The Trust has over 125 sites across South of England including Bexley, Bromley, Greenwich and Kent. The role will be based on Bexley clinical field within the Greenwich Community Diabetes Team.
