Job description

Bolton Hospital Foundation Trust are looking for a Diabetes and Endocrine Specialist Nurse, Band 7 for a full time post. This is a unique post where the successful candidate would work full time split over 2 specialities, working 2.5 days as a Diabetes specialist Nurse and 2.5 days as a Endocrine specialist nurse. Band 6 - 7 development would be considered for a suitable candidate.

Main duties of the job

To work as an autonomous, advanced practitioner and effectively deliver an expert service within the specialist area both from an inpatient and outpatient base.

To contribute to a high quality integrated Diabetes and Endocrine service that provides care for patients who have a diagnosis of Diabetes or established Endocrine conditions within primary, secondary and specialist care.

To provide specialist care to patients ensuring both the patient and their families are fully informed and supported.

To act as a resource and educator for both patients and staff in a hospital and community setting, providing advice and knowledge in managing these high risk groups of patients.

Maintain working relationships with the Diabetes and Endocrine MDT including Consultants, Specialist Nurses, Podiatrists, Dietitians, Support staff and clerical teams.
