Consultant in Integrated Diabetes Care & Weight Management

The Secondary Care Diabetes & Endocrinology service at Gloucestershire Hospital NHS Foundation Trust (GHFT), overseen by Clinical Lead Dr Alison Evans, delivers inpatient and outpatient care across all Trust locations. There is a full multidisciplinary diabetes team operating from our two main sites, and secondary care clinics in local community hospitals, which is complemented by close collaboration with the Gloucester Health and Care Community Diabetes Service and the Specialist Weight Management Service.

This is a newly established role designed to advance Type 2 diabetes care within Gloucestershire's integrated diabetes service, with a focus on promoting inclusive, person-centered care for patients living with diabetes. The successful candidate will be expected to lead the county-wide integration of diabetes care, utilising a population health management framework to address health inequalities and standardise care provision for individuals living with diabetes, particularly those living with a higher BMI.

The Trust provides a comprehensive suite of diabetes services, including antenatal care, insulin pump/Hybrid Closed Loop (HCL) management, multidisciplinary foot clinics and joint renal clinic. Inpatient care is based at Gloucestershire Royal Hospital within a dedicated specialty ward shared with Vascular Surgery. Notably, GRH serves as the regional vascular surgery hub, extending its specialised surgical support to the Swindon area.

Main duties of the job
  • Lead the ongoing integration of diabetes care.
  • Providing training, education, and support to professionals across the system, leading on the promotion of weight inclusive care and addressing obesity stigma.
  • Contribute to system wide pathway review and development.
  • Conduct largely community-based outpatient clinics for adults with complex Type 2 diabetes, young type 2 clinics.
  • Ensuring the best diabetes care for patients with a higher BMI.
  • Provide structured clinical governance to the Community Diabetes Specialist Team.
  • Collaborate with the multidisciplinary team (MDT) including Diabetes Specialist Nurses, Dietitians, Health Educators, Primary care Champion, and Podiatrists.
  • Work with the Acute Diabetes and Endocrinology team at Gloucestershire Hospitals NHS Foundation Trust.
  • Collaborative work with Specialist Weight Management Service to support patients living with obesity and to understand and promote weight management pathway in Gloucestershire.
  • Engage with Integrated Locality Partnerships and Primary Care Networks to develop the service.
  • Continue and develop MDT and mentored diabetes clinics focusing on education and obesity stigma within Primary Care Networks.
  • Involvement in various specialised clinics and education programs for diabetes.
  • Actively participate on audit and research.
  • There is no inpatient, GIM or endocrine commitment or acute on call cover attached to this role.

Qualifications and Training
  • Full General Medical Council registration
  • On Specialist Register for General (Internal) Medicine and Generic Medicine via CCT or CESR-CP (Combined Programme) (Proposed CCT/CESR-CP date must be within 6 months of interview)

  • Diplomas or postgraduate thesis relevant to the specialty
