The South East (SE) Neurosciences and Trauma Strategic Network is seeking a Complex Case Manager (CCM) to support the complex rehabilitation ODN and the Strategic Network in reviewing and transforming needs‑based complex rehab pathways across the South East. The successful candidate will work with two existing CCMs, with this role having a particular focus across Hampshire and the Isle of Wight. This role is unique in that it supports patients and their families in navigating the highly complex rehabilitation pathways and supports the Strategic Network in its transformation / streamlining of the pathways to improve access and outcomes for complex rehab patients.
