We have a rare and exciting opportunity for suitability experienced and enthusiastic Therapist who has appropriate experience and knowledge of post stroke care ideally in a community setting. Our multidisciplinary community stroke team provide rehabilitation support to patients in their own homes and bed bases across the footprint of CCICP utilising a Home First approach and proportionate care working closely with acute and community colleagues.
As Team Leader you will be based with the Community Stroke Team at Infinity House where you will be a visible and present leader who is professionally responsible and accountable for delivery of quality, safe patient care by therapists within your service, you will oversee staff development, education, shadowing and supervision and work closely with Community Operational Manager in maintaining and developing the service.
This post would be suitable for an NMC registered candidate or AHP.
CCICP adopts a Continuous Improvement approach, experience of this process would be advantageous for this role.
We are looking for an enthusiastic, dynamic and motivated Therapist to join our friendly and supportive Community Stroke Team to drive clinical excellence and service development.
You will lead, manage and co-ordinate the Community Stroke Service implementing and monitoring Trust policy, protocols and patient pathways working closely with referring hospitals and community services to ensure timely care and rehabilitation to post stroke patients.
You will work together with the Community Operational Manager to effectively manage budget and service development taking a proactive approach demonstrating positive leadership skills identifying and prioritising your workload.

To lead others from a base of clinical credibility to model and facilitate improved practice and partnerships.
Manage the day to day on going delivery and development of the service ensuring delivery of a service that is responsive, high quality, efficient which meets contractual obligations.
Work with the Operational Manager to ensure efficient and effective use of the workforce, implementing skill mix and staff deployment changes as appropriate to meet the changing needs/demands of patients.
