A local healthcare organization in Ashton-under-Lyne seeks an experienced nurse for a community care position within a Multi-Disciplinary Team. The successful candidate will assess patient care needs, develop care programmes, and ensure compliance with clinical procedures. You will coordinate case loads, support junior staff, and play a key role in the supervision of learners. The post holder will work within a Multi-Disciplinary Team in the Community and after a period of induction will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care, both for a specific group of clients as the named care provider and in supporting the care of all client groups within the community. You will act in accordance with locally agreed standards of care and ensure that clinical procedures are correctly carried out. As an experienced nurse and as appropriate, you will co‑ordinate the case load in the absence of the team/senior lead ensuring that effective communication is maintained at all times, providing support for junior staff within the team. You will pay a key role in the supervision and assessment of pre and post basic learners and unregistered staff providing a teaching role and acting as named supervisor.
