Job description
Overview

To participate in the provision of high quality care. This will involve assessing, supporting, and monitoring service users who are experiencing mental health issues. It involves working in a variety of community settings, liaising and co-working with other services (statutory and non-statutory), and engaging with carers and families in their homes, clinics, hostels, supported living, and residential settings.

Responsibilities
  • Work as a Care-coordinator or Lead Professional within the team in the planning, delivery, co-ordination, and evaluation of assessments and formulation.
  • Work closely and collaboratively with other team members including Therapists, Medical professionals, Nurses, Allied Health Professionals, and support staff.
  • Carry out complex person-centred assessments.
  • Ensure that all care offered is evidence based and that interventions take into account the physical, cultural, psychological and spiritual needs of service users.
  • Assess care needs, devise care programmes and packages of care including risk assessment, Dialog+ and implement and evaluate these in collaboration with individual service users and their carers.
  • Support and mentor less experienced staff, support workers, and students on placement within the team.
  • Undertake duties designed to extend clinical and managerial expertise as delegated by the Team Manager when required.
  • Undertake comprehensive outcome monitoring of all referrals.
