A leading healthcare provider in Doncaster is seeking a Care Coordinator to assess and support individuals experiencing mental health issues. This role involves working in community settings, collaborating with various professionals, and devising personalized care programs. Ideal candidates should have relevant qualifications and experience in mental health. To participate in the provision of high quality care. This will involve assessing, supporting, and monitoring service users who are experiencing mental health issues. It involves working in a variety of community settings, liaising and co-working with other services (statutory and non-statutory), and engaging with carers and families in their homes, clinics, hostels, supported living, and residential settings.
