Job Summary

The Dementia & Old People’s Mental Health Service in Mid Cornwall Locality of Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia and adults with functional mental health difficulties and increased complexity associated with physical ill health and the physiology of ageing.

This post is for a Community Psychiatric Nurse working with people who have a dementia illness and / or a functional mental health illness, the latter the age of 75 and over.

The post is based at St Austell, with the Fowey DOPMH Team, visiting people at home, in care homes and supporting our community health and social care teams.

The team in which you would be based has the following disciplines: A Consultant Psychiatrist, a Team Manager, a Memory Assessment Nurse, fellow Band 6 CPN’s, Primary Care & Dementia Practitioners, a Dementia Liaison Nurse, Health Care Assistants, and an Occupational Therapist.

You will be a qualified professional RMN and have experience of working with people with dementia and/or mental health disorders and be able to manage a caseload under the Care Programme Approach placing the person at the centre of care in a busy but rewarding role. The ability to work in partnership with carers/family members and services in the community is essential.

Main Duties

The post holder will provide assessment, treatment and care co‑ordination to two broad client groups:

  • People living with dementia.
  • Frail adults living with a range of mental health needs with the increased complexity associated with physical ill health and the physiology of ageing.

The post holder will work as a senior member of the team and will have the responsibility to work closely with a wide range of both statutory and non‑statutory agencies in developing and delivering personalised care under the care programme approach.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

  • Fal Team

It will be predominantly based in the Fowey Team but may require in‑reaching into other Mid Locality DOPMH CMHTs as service demands dictate.

Person Specification

Qualifications

  • Professional qualification - RMN, RGN
  • Current professional registration - NMC Professional Registration
  • Postgraduate qualification or equivalent experience in a psychological therapy e.g. CBT

Experience

  • Substantial experience of managing a busy caseload of clients in a community setting with Dementia and a range of mental health problems and frailty associated with old age
  • Providing clinical supervision
  • Experience of working in a multi‑disciplinary and multi‑agency environment

Knowledge and Abilities

  • A working knowledge of the Mental Health Act, the Mental Capacity Act, and Deprivations of Liberty safeguards.
  • A range of knowledge around physical health problems associated with ageing
