Overview

This post will include assessing older people referred to our service and working with an allocated caseload of existing service users, providing low level psychological interventions and/or memory assessments, and liaising with other professionals to coordinate care. You will need to safely administer medication in a community setting, engage in medicines management, and monitor physical health.

Responsibilities
  • Assess and support older people referred to the service with an allocated caseload.
  • Provide low level psychological interventions and/or memory assessments as appropriate.
  • Liaise with other professionals to coordinate and monitor care.
  • Safely administer medications in a community setting and engage in medicines management and physical health monitoring.
  • Work within a multidisciplinary and collaborative team, in the Team Base and in clients\' homes.
  • Assess risk dynamically and act to safeguard clients at all times.
  • Provide health promotion, psychoeducation, and educational input to service users, nursing students, and junior colleagues.
  • Be flexible and collaborative with the multidisciplinary team and external agencies.
Qualifications and Requirements
  • Be a registered Mental Health Nurse with a high level of commitment and motivation to work with older people with severe and enduring mental health problems and/or dementia.
  • Demonstrate excellent communication skills and the ability to provide clear information to service users about health promotion and psychoeducation.
  • Demonstrate the ability to safely administer medications in a community setting and undertake medicines management.
  • Be able to provide educational input and supervision to nursing students and junior colleagues.
  • Hold a full and valid driving licence and have access to a vehicle for work purposes due to the community element of the role.
