A prominent NHS Trust in Chertsey is looking for a Community Forensic Mental Health Practitioner to coordinate patient care and conduct mental health assessments. This Band 5 role provides a development pathway to Band 6 within 12 months, emphasizing collaborative work with a multi-disciplinary team. Candidates must have a relevant professional qualification, at least 6 months of post-qualification experience in a mental health setting, and a driving license. To provide specialist assessment and treatment for people with mental disorder, who have offended or are at risk of offending. The post holder will have a key role in the care coordination of patients within the community setting as well as Forensic assessment for patients in the community and in‑patient setting.
