Job description

To providea high level of clinical expertise, to support and facilitate individualisedpatient centred care to patients with a life limiting illness and to providecare and support for relatives and carers in collaboration with themulti-disciplinary team.

Tofacilitate and support the implementation of evidence-based practice andcontribute to the on-going development of the Service.

Toparticipate in a seven day per week operational service.

Main duties of the job

Demonstratesleadership through clinical expertise and specialist knowledge, delivering highstandards of personalised care.

Identifyingneeds through a holistic assessment and co-creating a care and support planwith people living with cancer and other life limiting illnesses.

Toindependently manage own caseload of patients and associated responsibilities.

Toindependently manage the role of duty nurse on rotation with colleagues.

To assess,and triage new patient referrals when required, in the absence of triagespecialist nurse.

To assess,plan, implement and evaluate all aspects of the patients care.

To providesymptom control, social, emotional, and spiritual care for the patient, family,and carers.

To providesupportive care to patients, their families, and carers.

To monitorpatients on an ongoing basis, to evaluate outcomes and adjust care plans toensure optimum co-ordination and delivery of care.

To deliverhigh standards of care directly and assist in the management of junior staff.

To developand maintain effective relationships with all members of the multi-disciplinaryteam to ensure collaborative working for the benefit of patient care.

To supportthe development and implementation of care pathways and patient information.

Tomaintain accurate and up to date patient records.

To ensurethat the dignity, safety, and confidentiality of all patients is respected andthat all patients receive the highest standards of care.

The Hospice alsoprovides support to their carers and families in patient homes, in care ornursing homes, or through its In-Patient Unit in the Hospice, as well as thoughthe Bradbury Wellbeing Centre. This roleis an essential component in supporting and complementing the Hospice CommunityServices Team, ensuring that the highest standards of care are implemented andthat services meet statutory regulations and national standards.

  • Experience of nurse management
  • Experience of working in a multidisciplinary team
  • Knowledge of palliative care and updated clinical practice within palliative care
  • Knowledge of national, regional, and local policy in relation to quality standards, targets, and improvements initiatives
