Job description
Overview

To be a core member of a multi-disciplinary Community Neuro Rehabilitation Team (CNRT) and contribute to the effective assessment and discharge planning of patients with stroke or other neurological conditions in both the area of Torbay and Teignbridge.

To undertake conversations to identify needs and risk and collate accurate and appropriate information to refer to the multi- disciplinary team or other Community Led and Voluntary/ Statutory services.

Under distant supervision, you will provide support in Torbay and South Devon after discharge to from hospital. This can include, but is not limited to; case managing those on the discharge to assess (D2A) pathway, recognise a change in care needs, carer support, financial advice, community access, holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust policies and guidelines.

This post is an exciting opportunity to work across both Torbay and Southern Devon NHS Foundation Trust, and Devon County Council. To undertake holistic needs-led assessments in partnership with service users, patients and carers, prepare care plans, commission services and manage a caseload to ensure the most effective and beneficial care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements. The post holder will need to ensure that their own contribution to service is delivered in a way that meets the diverse population of Southern Devon and adhere to policies on equality and diversity.

We have bases in both Newton Abbot Hospital and Paignton Health and Well Being centre and provide rehabilitation to patients following stroke or with neurological conditions. The team covers the Torbay and South Devon patches reaching from Dawlish to Dartmouth. We are supportive team who are passionate about delivering high quality care to our patients.
