Job description
About the role

This is an exciting opportunity for an enthusiastic and dynamic Care Coordinator to join the LEO Community Team. The post is open to nurses, OTs and social workers.

Responsibilities
  • Assessment: Conduct comprehensive assessments of newly referred adults with First Episode Psychosis and present findings to the wider team. Undertake risk assessments regularly and update care plans.
  • Engagement: Assertively outreach difficult to engage clients and meet service users at venues of their choice to support engagement and independence.
  • Support: Provide ongoing care coordination for a designated caseload of service users experiencing their first episode of psychosis, focusing on recovery and informed choice.
  • Interventions: Deliver low‑intensity CBT, Mindfulness, psycho‑education, and resocialisation interventions sensitive to BAME needs.
  • Families and Carers: Involve families and carers in treatment plans, assess their needs and provide psycho‑education.
  • MDT Working: Collaborate with the multi‑disciplinary team, attend clinical meetings, and liaise with inpatient services for early discharge.
  • Communication: Maintain accurate clinical documentation and ensure effective verbal and written communication with all involved parties.
  • Personal Development: Keep up to date with best practice, complete mandatory training, undertake CPD, and supervise junior staff members.
  • Miscellaneous: Participate in duty system, cover for colleagues, audit/clinical governance, research, and other duties as requested by the Clinical Service Lead.
Qualifications

Experience registered professionals with a specific interest in working with people developing psychosis. Evidence of completed preceptorship or band 6 skills is required.

About the Team

LEO is a multi‑disciplinary team providing a comprehensive service to people who may be experiencing psychosis for the first time, living in Lambeth. Successful applicants will be enthusiastic about this client group and committed to delivering NICE‑concordant care.
