Overview

We are looking to recruit a Care Coordinator to join Unique Community Services, a brand of Catalyst Care Group. The role is office-based during the initial 6-month period, with the prospect of transitioning to a hybrid work arrangement.

As a Care Coordinator, you will offer support and interaction to clients and clinicians within the organization, aiming to exceed client aspirations by delivering quality service. The role focuses on providing staffing solutions for client account management issues in collaboration with the Transforming Care teams, Account Management, and Sales, to promote client retention and development.

You will manage all incoming inquiries, working with team members to ensure they are addressed quickly and in a manner that accommodates client needs. The Care Coordinator will ensure precise staffing solutions that match client requirements to clinician capabilities, in adherence to current guidelines, policy, procedures, agreed-on standards, legislative requirements, and relevant regulations.

Is this you?

  • At least NVQ level 2 in Health and Social Care (desirable).
  • Driver’s license.
  • Previous experience in a senior role within the healthcare sector, or in positions such as Care Coordinator or Team Leader.
  • Previous experience dealing with or within a high-volume incoming call and query environment.
  • Experience using business software such as Microsoft Word, Outlook, and Excel.
