A local healthcare provider is seeking a proactive individual to coordinate and navigate care for patients, including the elderly and those with long-term conditions. The successful candidate will manage a caseload, working closely with GPs to ensure patients access appropriate services. Essential qualifications include experience in health and social care and IT proficiency. This full-time, permanent position offers a salary of £25,000 annually.
Maidenhead Primary Care Network, is a group of 9 GP Practices working together with a range of local providers to offer more personalised and coordinated health and social care to the 75000 patient population of Maidenhead in Berkshire.
The successful candidate will play a key role in proactively identifying and working with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They will work closely with GPs and practice teams, ensuring that appropriate support is made available to people; supporting them to understand and manage their condition and ensuring their changing needs are addressed. They will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve peoples' quality of life. They will work alongside social prescribing link workers and health and wellbeing coaches to provide an all‑encompassing approach to personalised care and enable people to navigate through the health and care system.
