A community-focused healthcare provider in Bury St Edmunds seeks a Care Co-ordinator to manage patient discharges from medical wards. This role requires ability to work in a team, relevant qualifications, and a commitment to patient confidentiality and care. The successful candidate will collaborate closely with ward staff and operational teams to facilitate smooth transitions for patients. An exciting opportunity has arisen for the recruitment of a Care Co-ordinator within the Medicine Division. The post holder will be an integral member of the team responsible for managing and facilitating patient discharges from the medical wards.
The successful post holder will work closely with ward staff, divisional operational teams, patient flow and Discharge Planning within West Suffolk Foundation Trust as well as wider social services department's
