Job description

We are looking for a BankCommunity Nurse to join our friendly, inclusive and forward‑thinking District Team, working across the West Lancashire area to provide care and support to patients in community settings.

The team is based out of Clayton House in Burscough. We have a variety of shifts available to fit around you and your personal life – the choice is yours! Please note that this role requires a full driving licence as it is community based.

Responsibilities
  • Work as part of the 24‑hour district nursing service, under the direction of the District Nursing Team Manager / Leader, providing high‑quality holistic care to patients.
  • Carry delegated responsibility for the assessment, planning, implementation and evaluation of evidence‑based nursing to patients in their own homes.
  • Assume delegated responsibility for the supervision of the team in the absence of the District Nursing Team Leader, when required.
  • Assist with the creation of a roster up to six weeks in advance, helping promote work‑life balance and meet personal commitments.
  • Support nursing and quality initiatives, including innovation and service redesign, and participate in quality improvements, research and development activity.
Qualifications & Experience
  • A Degree/Diploma in Adult Nursing
  • First‑level registered nurse
  • Experience working as part of a multi‑disciplinary team / multi‑agency working (Desirable)
  • Understanding of undertaking complex assessments and the planning and evaluation of care package
  • Excellent communication, both written and verbal; evidence of effective interpersonal skills
  • Clinical risk assessment skills
  • Ability to problem‑solve and make clinical decisions
  • Understanding of NMC standards and implications for practice
  • Ability to work independently in a community setting with high clinical standards
