Bank Band 5 Registered Nurse - North Wiltshire

We have an exciting opportunity for a Bank Registered Nurse Band 5 to join our North Wiltshire Locality Hospital at Home service. Our service is currently going through a positive period of transformation, making this a wonderful time to become part of a supportive, dynamic, and forward‑thinking team. The post will be based at either Chippenham, Malmesbury or Savernake Hospital, working Monday - Sunday 8am - 6pm (would consider shorter shifts).

As a Bank Band 5 Registered Nurse, you will be a valued member of the Clinical Team, working across organisational boundaries to bring together the strengths of both acute and community services. Our Step‑up team focuses on helping people avoid unnecessary hospital admissions, while our Step‑down team supports individuals to leave the acute setting safely and earlier when appropriate.

In this role, you will join a group of caring, innovative colleagues who are dedicated to the wellbeing of the local community. Together, you will help ensure that those who use our services receive high‑quality, compassionate care that truly makes a difference.

Main duties of the job

As a Nurse, you will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician. You will be an important part of the team, helping to meet the needs of patients in community settings, initially focusing on those referred through the 2‑hour Urgent Community Response (UCR), and later supporting the development of broader referral pathways.

Working within a multidisciplinary team and across primary, secondary, acute, and social care services in Wiltshire, you will play a key role in coordinating specialist interventions for people who might otherwise require hospital care, or who need timely support to leave emergency departments safely.

You will take responsibility for your clinical caseload and help ensure smooth coordination of care throughout each patient's journey.

Job responsibilities

As a Bank Registered Nurse Band 5, you'll be part of our valued team in our North Locality Hospital at Home service.

  • Self‑booking and access to all bank shifts! Staff are given access to all Bank shifts within their Business Unit and can self‑book providing they meet the shift requirements.
  • Request a Duty Staff can make shifts or Day Off requests prior to the creation of a roster up to six weeks in advance. This helps promote a better work life balance and allows for personal commitments to be met.

  • Able to work autonomously and as part of a team.
  • Experienced in the use of NEWS2 diagnostic tool and when to elevate.
  • Contribute to the development of others, supervising and teaching, mentoring, including student Nurses.
  • Previous experience in elderly care, frailty, community services or relevant acute care speciality.
  • Excellent written and verbal communication, and interpersonal skills.
  • Ability to adapt to the demands of a constantly changing environment.
  • Work within the NMC code of conduct.
  • Knowledge of community environment.
  • Good Communication skills including IT literacy and knowledge of computer systems.
  • Undertake evidence‑based clinical practice.
  • Experience of caring for people with long‑term conditions.
  • Experience of pressure ulcer prevention.
  • Awareness of self‑neglect, safeguarding and abusive situations to be the patient's advocate.
  • Ability to be an effective team member. Awareness of team dynamics.
  • Work within the boundaries of legislation, policies, procedures and standards.
  • Knowledge of clinical systems such as Systemone.
  • Other requirements: clean driving licence, car driver, access to a vehicle for daily use.
