Main duties of the job
  • Conduct initial mental health assessments using appropriate tools and frameworks.
  • Formulate clinical impressions and care needs based on assessment findings.
  • Assess and manage clinical risk, including safeguarding concerns.
  • Develop and review Dialog plus in collaboration with service users and carers.
  • Deliver therapeutic interventions appropriate to the service user's needs.
  • Liaise with GPs, social care, and other agencies to coordinate care.
  • Monitor and support service users\' physical health needs.
  • Maintain accurate and timely clinical documentation.
  • Apply relevant legislation including the Mental Health Act and Mental Capacity Act.
  • Promote recovery-focused and person-centred care.

Join Our Dynamic Community Mental Health Team as a Band 6 Screener

Are you a passionate and experienced mental health professional looking to make a real difference in the community? We are excited to offer an opportunity for a Band 6 Screener to join our dynamic and supportive Community Mental Health Team (CMHT).

As a Band 6 Screener, you will play a vital role in the initial assessment and triage of referrals into the CMHT. You will be responsible for identifying the most appropriate care pathways for individuals experiencing complex mental health needs, ensuring timely and effective support.

This is a frontline clinical role that requires strong decision-making skills, compassion, and the ability to work collaboratively within a multidisciplinary team.

Qualifications/ Registrations
  • Registered Mental Health Nurse with current registration with NMC.
  • Evidence of ongoing professional development and relevant post-registration training.
  • Demonstrate a clear understanding of the Mental Health legislation or social care legislation relevant, its application and use in practice.
  • Experience/ Knowledge
  • Ability to communicate effectively within a multidisciplinary team. Skilled in verbal, non-verbal, and written communication, especially when dealing with sensitive or complex information.
  • High-level clinical assessment skills to screen and triage patients effectively.
  • Ability to interpret complex health and social needs, including test results and patient histories.
  • Must have experience in coordinating care, managing caseloads, and making informed decisions based on clinical judgement.
  • Full driving license and access to own vehicle.
  • Proficient in using electronic patient records, digital communication tools, and data entry systems.
  • Ability to work within teams and manage own workload.
  • Experience in supporting junior staff, students, or new team members.
  • Demonstrates the ability to lead on service improvements or contribute to team development.
  • Shows initiative in identifying areas for change and implementing solutions in digital triage platforms.
  • Must have community experience.
