Are you ready to embark on an exciting journey as a Band 6 Physiotherapist with our dynamic Discharge to Assess team at Livewell Southwest? This locum role offers a rewarding chance to make a significant impact by supporting timely and safe patient discharges. Join us in this full-time position, earning £26 per hour and start ASAP for a period of three months. Use your expertise to carry out comprehensive assessments and work collaboratively as part of a multidisciplinary team to optimise patient outcomes.

Responsibilities
  • Undertake timely and effective physiotherapy assessments for patients referred to the Discharge to Assess service.
  • Develop and implement evidence-based treatment and rehabilitation plans to promote patient wellbeing.
  • Collaborate effectively with the multidisciplinary team to facilitate safe, efficient, and appropriate patient discharges.
  • Set realistic and achievable patient-centred goals to enhance independence and recovery.
  • Monitor, evaluate, and adapt treatment programs to meet patient needs and progress.
  • Assess patients’ functional abilities, providing recommendations for mobility aids, equipment, or community services as required.
  • Contribute to discharge planning through accurate documentation and effective communication with stakeholders.
  • Support the coordination of care across health and social services to reduce the risk of hospital re-admissions.
  • Offer professional advice and guidance to patients, families, and carers on rehabilitation and self-management strategies.
  • Maintain accurate, up-to-date clinical records consistent with professional and service standards.
