In this role, you’ll coordinate care with shared living providers, manage financial needs, medical appointments, shopping, and other essentials. You’ll also work with the Department of Developmental Services (DDS) to complete documentation for Individual Support Plans (ISP) and medication management. Requirements
  • Bachelor’s degree in human services/related field preferred
  • Experience in direct support for developmental disabilities is a plus
  • High School Diploma or GED
  • The ability to physically intervene in instances of extreme behavior that might jeopardize the safety of students and/or staff.
  • Valid driver's license
  • Strong organization, communication, and teamwork skills
  • Ability to work independently
  • Flexible schedule, including on-call coverage
Schedule

40 hours a week. Monday - Friday 9am - 5pm

Compensation

$52,000 - 62,000 per year

Responsibilities
  • Work with the SCL Manager to maintain accreditation and state compliance.
  • Provide non-violent crisis intervention as needed.
  • Complete ISP documentation and attend annual ISP meetings.
  • Assist with program development, implementation, and design.
  • Oversee financial records and money management plans.
  • Submit monthly financial packets per LCI procedures and timelines.
  • Provide case management to ensure a safe, healthy home environment.
  • Arrange transportation for medical appointments, job interviews, and community access.
  • Assist with personnel management, including training, supervision, and performance input.
  • Coordinate with nursing staff for medication and medical care oversight per DDS/MAPs.
  • Document case management and support activities in Therap.
  • Record behavioral incidents and reportable events in Therap (GER).
  • Attend team meetings as requested by the SCL Manager.
  • Help plan and schedule healthcare, vocational, and social activities.
  • Support individuals with ADLs, personal needs, and community involvement.
  • Perform additional duties as assigned, except for occasional emergency tasks outside normal duties.
