Overview

Essex Partnership University NHS Foundation Trust is recruiting an Ambulance Triage Nurse (Ambulance Support) to join a fast paced urgent community care pathway providing an urgent and crisis response to those experiencing mental distress within the community. You will work jointly with the Ambulance Paramedic clinician by providing immediate crisis care in the community across the Mid and South Essex. You will support other Paramedic crews in managing patients with complex mental health needs. You will work with non-statutory and statutory services to support people while in crisis.

Main duties of the job

You will conduct initial assessments, agree formulation of need and develop collaborative care plans underpinned by NICE Guidance delivering a range of interventions based upon individual needs. Assessments and interventions can take place in patient\'s homes, team bases, hospital and other community settings or via tele-coaching and through the use of assisted technology and online forums. The successful candidate will be working within the Mid and South Essex area conducting assessments and interventions over a twenty four hour period.

Job responsibilities

The successful candidate will be working within the Mid and South Essex area conducting assessments and interventions over a 24 hour period.

Work flexibly within the team accompanying colleagues for face to face support to Ambulance Paramedic crews in service users homes

Provide and oversee the management and care of patients

Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your professional Code of Conduct and Trust policy

Person Specification
Education/Qualifications
  • Registration (Mental Health Nurse, Occupational Therapist or Social Worker)
  • High standard of education
  • Post graduate qualification or commitment to work towards this
Knowledge
  • Proven experience of working in a health, social care or community setting as part of a multi-disciplinary team
  • Experience of dealing with confidential and sensitive information
  • Proven experience of working with people with mental health conditions and vulnerable service users
  • Proven experience of conducting and contributing to ongoing assessments
Skills/Experience
  • Literate in IT/Computer Skills
  • Good oral communication skills based on fluency on the English language
  • Ability to work with Clinical Manager in the development and implementation of operational / Trust policies
  • Assess and treat own caseload of service users in accordance with Trust clinical pathways
  • Undertake physical health checks of services users and act on the findings appropriately
  • Support families and carers within the boundaries of their role and professional code of conduct
