A healthcare provider in the UK is seeking an Advanced Clinical Practitioner to conduct comprehensive health assessments for adults and children, both face-to-face and over the phone. The role requires full NMC registration and at least two years of experience in a GP Practice. You will undertake comprehensive health assessments both face to face and over the telephone, including adults and paediatrics; practicing autonomously whilst demonstrating a person-centred approach to care delivery.Within your professional boundaries you will assess, diagnose, plan, implement and evaluate treatment/interventions. Provide care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.You will practice autonomously and prioritise, organise and manage own workload in a manner that maintains and promotes quality.Take responsibility for own learning and performance, acting as a positive role model.

Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are excited to share that we are looking for an ACP to work within our GP Practices in Primary Care. We have set shifts available over our two clusters around Merseyside.
Location:
Cluster 1 15 Sefton rd.,Litherland, Seaforth, Netherton health centre.
Cluster 2 Maghull, Hightown, Thornton Health centre, Crosby/Crossways, North Park.
