Qualifications

• Previous experience in a social care, community support, or health-related role.
• Understanding of adult safeguarding, Mental Capacity Act, and the Care Act 2014.
• Strong communication and organisational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Confident in building relationships with adults, families, and partner agencies.

Responsibilities

• Support Social Workers by gathering information and assisting with assessments.
• Help develop and review care and support plans.
• Liaise with families and partner agencies for coordinated support.
• Assist with arranging services like home care and community resources.
• Contribute to safeguarding enquiries by reporting concerns.
• Support timely hospital discharges with care packages.
• Maintain clear and accurate case notes.
