Overview

Please note this vacancy holds 3 x 3 month fixed term contracts and 1 x 6 month fixed term contract. The post holder will work as part of a team providing highly visible support to all areas and personnel to assist in the delivery of safe, appropriate and effective care for acutely ill and deteriorating patients. They will function in a supportive and educational capacity in the clinical environment supporting ward teams in ensuring the patient receives the right care at the right time in the right place.

Responsibilities
  • To ensure that patients with clinical deterioration have their needs prioritised with appropriate assessment, intervention and on‑going management. To ensure that patients at risk are reviewed regularly and appropriate intervention initiated in a timely manner.
  • The post holder will assist with the delivery of education through enhancing staff confidence, competence and experience of ward staff through sharing of skills.
  • The service will utilise physiological track and trigger tools to identify patients whose condition is deteriorating via the Electronic patient record.
  • They will assist/support audit projects and update guidelines/policies within the Acute Intervention Nurse remit.
  • Assist / Support ward staff with care of the dying patients, complex DNACPR decisions and ensure effective communication with the patient and their families.
  • To assist the anaesthetic team to stabilise critically ill patients throughout the hospital.
  • To be an active member of emergency response teams within the hospital, taking a lead role in the Medical Emergency Team.
  • Capable of acting as team leader in the Cardiac Arrest Team when required.
  • Active member of the Paediatric Emergency Team

Qualifications and Other Information

You must be able to produce ALL certificates stated essential in the person specification or you will not be able to complete pre-employment checks. We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le‑Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
