POSITION SUMMARY
Our client is currently seeking a Registered Practical Nurse for their Hospital & Nursing Department. The Registered Practical Nurse is responsible for assessing, planning, implementing, and evaluating nursing care for individual patients and families. The nurse assigned to this position is required to function independently and collaboratively with physicians and the multidisciplinary team in the clinic to ensure efficient clinic operations.
Availability to work days, evenings, and weekends is expected. This role involves working within an integrated, multidisciplinary team supporting children, youth, and families to access and navigate services.
The nurse will be familiar with workflows related to referrals, appointment scheduling, work queue management, specialized services within Ambulatory Care, and community resources to support each child's, youth's, and family's unique needs.
The nurse will address barriers to service/care, support the patient’s journey by partnering with others for care coordination, and provide education and resources. This role promotes quality improvement, customer service, safety initiatives, and strives for optimal performance and safety benefits for staff, patients, and their families.
This position may require regional travel and community-based visits with patients and families.
MAIN RESPONSIBILITIES
- Assess the patient (e.g., lab results, x-ray, pathology, clinical data, vital signs, measurements, specimen collection).
- Assist the team with procedures and monitor patients pre and post-procedure as required.
- Administer medications within scope of practice.
- Assist with managing work queues and triaging new referrals for urgency.
- Receive calls and virtual communications from parents; obtain information and respond or redirect inquiries as needed.
- Coordinate care with home care and other healthcare professionals and specialists.
- Provide instructions (written or oral) to patients and families, including teaching procedures, specimen collection, or discharge instructions.
- Provide emotional support to patients and families.
- Maintain documentation according to hospital policies and procedures.
- Maintain equipment and report malfunctions.
- Order and restock supplies, check expiry dates on supplies and medications.
- Check resuscitation cart regularly.
- Possess a valid driver’s license and access to a vehicle.
- Note: Tasks may be added or removed as the role evolves.
QUALIFICATIONS
- Vulnerable Sector Check (Essential)
- Current registration with the College of Nurses in Ontario (Essential)
- Current BCLS certification (Essential)
- Medication administration course (Essential)
- Knowledge of safe medication practices and reconciliation in a clinical setting (Essential)
- Excellent interpersonal skills (Essential)
- Ability to participate as an active healthcare team member (Essential)
- Effective communication skills in oral and written formats with patients, colleagues, and other health disciplines (Essential)
- Willingness to participate in self-evaluation (Essential)

- Understanding of aseptic technique and ability to apply in clinical practice (Essential)
- Ability to maintain clinic throughput through nursing functions such as rooming patients, assessments, and documentation (Essential)
- Bilingual in French and English (Essential)
- Minimum two years pediatric experience (Preferred)
- Ambulatory Care experience (Preferred)
