Mass General Brigham Home Care serves patients recovering from illness or injury, as well as those managing chronic diseases. Responsibilities

• Provide physical therapy services to patients in their homes, addressing physical impairments, functional limitations, or disabilities caused by injury, disease, or other factors. Patients range from birth to over 100 years old.
• Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate appropriate services.

Key Activities

• Develop patient- and family-centered care plans with standardized interventions aimed at goal achievement.
• Ensure daily documentation is accurate and timely.
• Apply standard treatment procedures.
• Educate patients and caregivers.

Qualifications and Experience

• Minimum of one year of experience in acute care, rehabilitation, or home care as a Physical Therapist.
• Registered with the Massachusetts Board of Registration.
• Strong customer service skills and ability to work with diverse populations.
• Excellent verbal, written, and electronic communication skills.
• Proficiency with computers and ability to learn new software.
• Ability to prioritize tasks and adapt to changing circumstances.
• Willingness to travel within the Mass General Brigham Healthcare at Home area.
