Job summary

The Opportunity

The Social Worker assists residents and their families by providing support and assessment for residents, related but not limited to, move-in process, transitions, finances, concerns and decline in health as well as assistance accessing community resources.

What Is In It For You
  • Assists the Executive Director and Office Manager with the process of moving new residents into the home by coordinating with incoming residents and their Power of Attorney (POA) and providing support with aspects of the move-in process that require the involvement of the Social Service Worker.
  • Participates in the residence’s Continuous Quality Improvement (CQI) program.
  • Evaluates the effectiveness of Social Service Work services on an annual basis and provides input to update policies and procedures that impact the Social Service Work practice.
  • Prepares and submits reports to the residence and/or corporate office as required.
What You Will Need
  • Bachelors or Master\'s Degree in social work.
  • Minimum one-year relevant experience preferably in a health care setting.
  • Previous experience in a long-term care setting is preferred.
