Job summary

Overview

The Social Service Worker assists residents and their families by providing support and assessment related to move-in processes, transitions, finances, concerns, health decline, and accessing community resources.

What Is In It For You
  • Assist the Director of Care and Business Manager with the process of moving new residents into the home by coordinating with incoming residents and their Power of Attorney (POA) and supporting aspects of the move-in process.
  • Participate in the residence’s Continuous Quality Improvement (CQI) program.
  • Evaluate the effectiveness of Social Service Work services annually and contribute to updating policies and procedures.
  • Prepare and submit reports to the residence and / or corporate office as required.
What You Will Need
  • Diploma in Social Service Work.
  • Minimum one-year relevant experience, preferably in a healthcare setting.
  • Experience in a long-term care setting is preferred.
