Job summary
A community support organization located in Canada is seeking a dedicated individual to assist clients with their rehabilitation and life skills development. The role involves supporting clients in achieving their Individual Support Plans, managing challenging behaviors, and engaging them in community activities. Ideal candidates should be compassionate and have experience working with individuals with complex needs.
Responsibilities
- Independently work with clients to achieve goals in their Individual Support Plan.
- Support clients with complex behavioral, emotional, and social needs.
- Use de-escalation techniques for challenging behaviors.
- Assist clients in developing life skills like meal prep and cleaning.
- Model healthy relationships and set boundaries.
- Drive clients to appointments and participate in recreational activities.
Responsibilities
- Independently work with client(s) towards achieving the goals set out in their Individual Support Plan (ISP); encouraging them during setbacks and celebrating successes with them.
- Support / supervise complex behavioural, emotional and social needs / development (in the residence & during community activities).
- Use de-escalation techniques to deal with challenging behaviours.
- Assist client in developing life skills within the home including household chores (meal prep, cleaning, etc.) in a consistent, healthy & safe environment and their community.
- Model healthy relationships and set boundaries.
- Drive person in care (in your own or company vehicle), accompany to appointments, and supervise / participate with the client in recreational and community activities.
