Qualifications

  • Registered and in good standing with the College of Nurses of Ontario.
  • Valid CPR HCP certification.
  • Minimum 2 years of clinical experience in a community healthcare setting.
  • Experience in chronic disease management and health teaching.

Responsibilities

  • Implement and coordinate management system for Chronic Disease Management.
  • Provide clinical support within a multidisciplinary team.
  • Engage in health promotion/teaching activities.
  • Maintain and manage patient files while prioritizing confidentiality.
