Qualifications

• 3-5 years clinical experience working as a Registered Nurse.
• Recent experience in Pediatrics, Chronic Disease Management, Medicine, or Cardiology preferred.
• Advanced proficiency in English and French.

Responsibilities

• Provide first in-home nursing visit within 24 hours of hospital discharge.
• Perform clinical assessments and medication reconciliation.
• Assist patients in understanding their care plan and treatment.
