Position Summary

The Physiotherapy Assistant/ Occupational Therapy Assistant will engage in community work that involves commuting across the Southeastern Ontario region. The Transitions Home Program goal is to achieve timely access to care and clients’ functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client’s home and community settings.

Under the general direction of the Program Manager, the Physiotherapy Assistant/Occupational Therapy Assistant (PTA/OTA) is responsible and accountable to provide person-centered care for assigned patients/clients under the direction and supervision of the regulated health professional (supervising clinician) and performs associated functions to support service delivery.

As a member of a team, the PTA/OTA participates in profession-specific and interprofessional activities that are directed towards a person-centered environment and contribute to operational efficiency and professional and program development.

Within this role, the PTA/OTA will work autonomously in a variety of settings providing services to patients/clients and families by following the treatment plan developed and assigned by the supervising clinician. Thorough knowledge of scope of practice and of the role and standards of practice of Physiotherapists and Occupational Therapists is critical in this role.

Qualifications
  • Dual Physiotherapy Assistant/Occupational Therapy Assistant certificate from a recognized Physiotherapy Assistant/Occupational Therapy Assistant Program
  • Valid driver’s license
  • Minimum six months of experience in each of OT and PT settings, preferred
  • Previous experience with target populations such as Complex Medical Care, Palliative Care, Seniors Rehabilitation and/or Rehabilitation, Seniors Mental Health, Adult Mental Health, Forensic Mental Health, preferred
Position Responsibilities
  • Follows assigned treatment plan independently for patients/clients (individual or group), including the use of skilled techniques as deemed appropriate by the supervising clinician
  • Collaborates with the clinician on individual or group activities within the pre-determined guidelines of the assigned treatment plan
  • Discusses and clarifies patient/client needs and brings forward suggestions in day-to-day problem-solving
  • Fits and instructs safe use of assistive devices and walking aids, as per supervising clinician assists in fitting adaptations or aids and fabricates treatment aids or adaptations for equipment as required
  • Provides exercise programs to assigned patients/clients as per the direction of the supervising clinician
  • Regularly monitors and reports patients’/clients’ responses to treatment and to supervising clinician; suggests changes/adaptations to the treatment plan
