Qualifications

• 3-5 years experience in Long Term Care, experience with MDS an asset.
• Knowledge in nursing rehabilitation and working OT and PT.
• Ability to lead training and education with Adult learners.

Responsibilities

• Coordinate the completion of MDS assessment on all residents.
• Understand and provide critical analysis of data at different levels.
• Act as key contact and liaison with external health entities.
