Qualifications

• RN / RPN Degree or Diploma from a recognized college or university with CNO registration in good standing.
• Minimum of 2-3 years experience in community health preferred.

Responsibilities

• Initial visit and thorough assessment to establish care plan.
• Ensure clients' concerns and complaints are addressed.
• Provide teaching related to special functions.
• Collaborate with clients and care team to create care plan.
• Maintain documentation standards according to policies.
