We are seeking a bold, collaborative and values-driven leader to join our Senior Management Team as Director, Primary Care.

The Opportunity:
Reporting to the Chief Executive Officer, the Director, Primary Care will directly supervise two Primary Care Managers and an inter-disciplinary team, overseeing both day-to-day operations and long-term systemic change. You will play a central role in shaping organizational strategy, advancing health equity, and ensuring the sustainability and impact of CCHC’s work. This is a chance to transform how health care is delivered and experienced – locally and systemically.
Requirements
Our ideal candidate has a(n):
- Master’s degree in Health Administration, Health Care, Social Sciences, related discipline or equivalent combination of education and experience.
- Minimum five (5) years progressive management experience, with relevant clinical experience, managing a multi-disciplinary health care team including supervision of staff.
- In-depth understanding of community-based health care, equity-informed approaches, and the social determinants of health.
- Demonstrated leadership experience in addressing issues of inequity, racism and oppression on an individual, organizational and systemic level
- Exceptional interpersonal, communication, program development, evaluation, quality improvement experience and partnership-building skills.
- Commitment to advancing digital health, interdisciplinary practice, and integrated care models.
- Understanding of the standards of the College of Physicians and Surgeons and the College of Nursing of Ontario.
- Proficient with Windows-based systems/ Microsoft office suite
- Availability to work occasional evenings and weekends.
- Familiarity with management information systems
- Clinical designation (e.g. RN, RD, RSW/RP, OT, PT) a strong asset
