About the Role

Do you believe health equity is a right, not a privilege?

Are you passionate about leading with purpose, championing systemic change, and building healthier, more just communities?

At CCHC we don’t just deliver care – we work alongside the community to transform the conditions that shape health. As a non-profit multiservice Community Health Centre, serving more than 21,000 individuals each year, we are deeply committed to supporting those most affected by inequities: isolated senior population, the LGBTQ+ community, Indigenous Peoples, racialized communities, francophone, those affected by mental illness and addictions, people who experience homelessness, individuals and families on low income, and newcomers to Canada.

We are seeking a bold, collaborative and values-driven leader to join our Senior Management Team as Director, Primary Care.

Why This Role Matters:

  • As Director, Primary Care you will be both a strategic visionary and operational leader, guiding a team that delivers high quality, client-centred primary health care through the clinic and outreach services.
  • Shape the future of primary care at CCHC and across our catchment area of Centretown, the Glebe and Old Ottawa South
  • Build on CCHC’s reputation as a trusted advocate and innovator in community-based care
  • Build and nurture partnerships with hospitals, community agencies, academics and researchers, policy makers and peer CHCs to advance equity-driven policies and services
  • Continue to advance the CHC model as a cornerstone of Ontario’s health system.

Your Impact:

  • Lead with purpose: Inspire and support an interprofessional team of physicians, nurse practitioners, and allied health professionals.
  • Advance equity: Ensure services are designed and delivered with and for communities facing systemic barriers.
  • Influence systems: Represent CCHC and the CEO at decision-making tables, championing equity-driven policies and expanding services for priority populations.
  • Shape the future: Drive innovation in primary care through program evaluation, continuous improvement, and community collaboration.

Who You Are:

  • A values-driven leader who believes in justice, equity, and community voice.
  • A strategic thinker with experience building and leading high-performing teams.
  • A relationship-builder who thrives in partnership and cross-sector collaboration and supporting your dedicated team.
  • A change agent excited to strengthen CCHC’s role as a leading voice in health equity.
  • A steady and adaptive leader —able to manage complex operational demands without losing sight of long-term strategic goals.

The Opportunity:

Reporting to the Chief Executive Officer, the Director, Primary Care will directly supervise two Primary Care Managers and an inter-disciplinary team, overseeing both day-to-day operations and long-term systemic change. You will play a central role in shaping organizational strategy, advancing health equity, and ensuring the sustainability and impact of CCHC’s work. This is a chance to transform how health care is delivered and experienced – locally and systemically.

Requirements

Our ideal candidate has a(n):

  • Master’s degree in Health Administration, Health Care, Social Sciences, related discipline or equivalent combination of education and experience.
  • Minimum five (5) years progressive management experience, with relevant clinical experience, managing a multi-disciplinary health care team including supervision of staff.
  • In-depth understanding of community-based health care, equity-informed approaches, and the social determinants of health.
  • Demonstrated leadership experience in addressing issues of inequity, racism and oppression on an individual, organizational and systemic level
  • Exceptional interpersonal, communication, program development, evaluation, quality improvement experience and partnership-building skills.
  • Commitment to advancing digital health, interdisciplinary practice, and integrated care models.
  • Understanding of the standards of the College of Physicians and Surgeons and the College of Nursing of Ontario.
  • Proficient with Windows-based systems/ Microsoft office suite
  • Availability to work occasional evenings and weekends.
  • Familiarity with management information systems
  • Clinical designation (e.g. RN, RD, RSW/RP, OT, PT) a strong asset
  • English, written and spoken
