About the Role

The Community Support Worker - Trans Health Clinic is responsible for the intake and assessment of new clients into the Trans Health clinic and the coordination, both internally and externally, of team-based care for those clients expressing a need for support with social as well as medical transitioning goals. Additionally, the Community Support Worker - Trans Health collaborates with members of the Trans Health clinic team to strengthen and enhance pathways of care for Trans and gender diverse clients within the clinic setting and broader community.

Community Support Worker – Trans Health Clinic reports to Manager of the Mental Health and Substance Use Health and Addiction and is supported by the Program Lead - Trans Health Clinic.

Requirements
  • Bachelor’s degree in Social Work or social science or equivalent combination of education, lived experience and/or professional experience.
  • Minimum three (3-5) years relevant experience.
  • Designated position: Lived experience as Trans, Two-Spirit, Intersex and/or Gender Diverse
  • Deep understanding of Gender Affining Care (GAC) and how to implement best practice to health care and social practice
  • Demonstrated understanding of the systemic barriers Trans, Two-Spirit, Intersex and Gender Diverse individuals face in accessing health and social services; as well as strategies to overcome these barriers
  • Thorough knowledge of trans health resources within Ontario and other provinces as applicable
  • Thorough knowledge of processes related to referrals for trans care surgeries
  • Experience in crisis intervention/risk assessment, support and safety planning
  • Skilled in emotion regulation, crisis de-escalation and boundary setting
  • Demonstrated ability to work with clients with multiple, intersecting barriers
  • Demonstrated ability to work with individuals 17 and up; ability to apply a youth lens/focus when required
  • Demonstrated ability to work with systems of care (support systems; external systems etc.) in collaboration with the client to ensure a holistic approach to care
  • Demonstrated ability to work collaboratively across multidisciplinary teams
  • Knowledge of best practices related to health care for Trans, Two-Spirit, Intersex and Gender Diverse individuals
  • Sherbourne Guidelines and Protocols for Hormone Therapy and Primary Health Care for Trans Clients 2015
  • Informed Consent models and principles
  • WPATH Standards of Care Version 7, 2012
  • Demonstrated understanding of and experience with Motivational Interviewing
  • Demonstrated understanding of anti-racism/anti-oppressive perspectives and intersectionality
  • Demonstrated understanding of trauma informed care
  • Demonstrated cultural competency and flexibility in the provision of service to individuals from diverse cultures and backgrounds
  • Establishes a caring and supportive professional relationship with clients; demonstrates trust, respect, dignity and inclusion
  • Excellent verbal and written communication skills
  • Strong interpersonal skills
  • Ability to handle multiple tasks and prioritize assigned duties
  • Flexibility, good judgement and initiative
  • Ability to work evenings and some weekends
  • Proficient with Windows-based computer systems including email, MS Word and data base programs
  • Ability to meet deadlines and work within a team-based environment
