Job description

We currently have a vacancy for a Community Mental Health Nurse at the Heywood & Middleton CMHT. This post is a full time position and is initially a 3 month secondment.

You will need to be a registered and experienced RMN.

The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community.

The practitioner will also:Work collaboratively across a range of disciplines to continuously promote a 'needs based' approach to all service users.Promote an integrated approach to service delivery.Develop clear relapse recognition / prevention plans and share with MDT, service users and carers where appropriateDevelop meaningful engagement with individuals who are often referred to as 'difficult to engage' and their families.

Main duties of the job

The successful applicant will work effectively with adult service users who have mental health needs across Heywood and Middleton . The successful applicant will assume the role of a care coordinator and will manage the care of a defined caseload of patients.

The post holder will be required to provide a range of highly skilled interventions including group work and share those skills with other team members.

The CMHT work primarily with working age adults who have a diagnosis of a severe and enduring mental Illness and have complex mental health difficulties. The care pathway includes a holistic assessment of need, risk formulation and the delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning

Qualifications
  • Registered Mental Health Nurse / Occupational Therapist / Social Worker
  • Current/active registration with professional regulatory body
  • Qualification in psychologic therapeutic interventions
Experience
  • Substantial experience of relevant post registration practice
  • Experience of managing a caseload
  • Experience of working in a multi-disciplinary team setting
  • Experience of working with people with a range of severe and complex mental health problems
Knowledge
  • Good working knowledge of therapeutic assessment techniques and interventions in the management of behaviour of individuals with complex mental health needs

  • Comprehensive understanding of Safeguarding adults procedures and practice
