Qualifications

• Minimum of three years’ experience in community health center setting.
• Experience in chronic disease management, including diabetes and hypertension.

Responsibilities

• Provide both access and attachment to primary care within the scope of a Registered Nurse.
• Coordinate client care within the health centre and with specialists.
• Initiate health teaching and health promotion with clients.
